Psychosocial Assessments and Functional Capacity Assessments

Psychosocial Assessments

Our comprehensive psychosocial assessments evaluate your mental health, social environment, and overall wellbeing. This provides a clear picture of areas of concern and helps guide the right interventions and support strategies to improve your quality of life.

Functional Capacity Assessments

For individuals needing a detailed evaluation of their physical, mental, and emotional capacities, we offer functional capacity assessments. These assessments identify how psychosocial challenges impact day-to-day functioning, and are essential for accessing appropriate services and support — helping ensure you receive the resources you need to live independently and with dignity.

This assessment can also serve as important supporting evidence for NDIS (National Disability Insurance Scheme) funding applications, helping demonstrate the support and services needed to improve your quality of life and independence.

What to Expect

Assessments typically involve one to two sessions, combining interview, observation, and where relevant, structured assessment tools to build a clear picture of your circumstances and support needs. You’ll receive a written report within 2–4 weeks of your final session, which you can use to support NDIS applications, plan reviews, SACAT matters, or discussions with other services and providers. You’re welcome to self-refer, or you may be referred by your GP, support coordinator, solicitor, or another service. All information shared during the assessment process is kept confidential in line with professional and privacy standards, and reports are only shared with third parties (such as NDIS or SACAT) with your consent. Assessments are conducted by a qualified social worker with experience preparing reports for NDIS, aged care, and tribunal purposes. For information about fees, please contact us.
